Description

This session provides a framework for chronic pain assessment, intervention, and interprofessional collaboration in primary care. Evidence-based approaches that address multiple dimensions of pain and best practices for interprofessional collaboration will be described to promote unified, patient-centered care. The session will explore methods to support patients in building a personalized decision-making toolkit that draws on a spectrum of pain management strategies and resources. Participants will leave with skills to enhance clinical effectiveness and patient empowerment.
