Objectives:
• Demonstrate baseline knowledge of diabetes and hypertension in order to provide psychoeducation to patients
• Utilize a contextual interview to increase insight into contributing factors and possible areas of growth related to a person’s chronic disease
• Utilize MI to create a patient centered and directed treatment plan
• Create an action plan of ways to increase chronic disease handoffs as part of a care pathway
